Request an appointment

We offer a variety of appointment times so that you can find a time that fits your schedule.  Appointments are available Monday thru Saturday.  Appointment times vary by day and by counselor. We offer in person AND telehealth services.

You have several options for scheduling, canceling or rescheduling an appointment, including an initial consultation.

  • Use the on-line scheduler link below to request an appointment with a therapist.  The therapist will either confirm your appointment request OR will call you to discuss an alternative date and time.
  • Call us at 904-262-1900
  • E-mail us by using the contact form.
Once the appointment for your Initial Consultation is scheduled, you will receive an invitation to create an account in the Client Portal.  Please set up this account as soon as possible as this will allow us to send you the paperwork required for your appointment.  You will be able to complete this paperwork and return it to us through the portal.  No paper!  We use a HIPPA compliant service so no worries there.  It is important that you complete the paperwork prior to your appointment so that your appointment time can be focused on you and not on paperwork.  Additionally, you and your therapist will be able to exchange other valuable materials through the portal.
